We are so sorry you are going through financial struggles. Our goal is to help as many members as possible. We are, however, limited on the amount of funds we have available to assist those during these times.

To help us help you, please fill out this application completely. If you are filling this out online, please submit it when you have all the supporting documents. If you are filling out a hard copy, you may return this to the Campus Administrator that you received the application from.

Here are a few guidelines to remember about our assistance program:

• Each person would need to be a member of Liberty Live Church for 6 months prior to this request.
• We ask that you be active within the life of the church.
• After review of your finances, we may suggest some financial coaching if the situation will help you in the future.
• All applications will need to include a copy of the bill you are requesting assistance with.
• Any assistance offered cannot, per the IRS rules, be written to an individual. All checks are written directly to the company that corresponds with the bill submitted.
• Assistance is limited to one bill every 12 months.
• It may take up to 10 days to process this application so please make sure to plan ahead.
